Ryan White/HIV Services

CCHC Ryan White HIV/AIDS Program is funded to provide comprehensive HIV/AIDS related services for individuals who do not have adequate health care coverage or financial resources for coping with HIV/AIDS. Our multidisciplinary team comprised of primary care providers, dental, mental health professionals, nurses, patient advocates, case managers, pharmacy, health administrators, referral coordinators and community outreach specialists are committed to providing the highest standard of confidentiality and culturally appropriate patient-centered care.

The agency receives Ryan White Part A and B funding through the AIDS Foundation of Chicago as a sub-recipient. CCHC receives direct Ryan White funding from HRSA through Part C Early Intervention Services.

CCHC receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals. We have achieved NCQA Level 3 Patient Centered Medical Home recognition at all three clinic sites. We are IDHS-SUPR licensed to provide Level 1 and 2 outpatient substance use treatment services.
