Our Case Managers will engage clients to identify needs and create a client care plan. Case Managers keep in contact with clients to help them meet their health goals.
Once care plans are created, Case Managers work with their clients to reduce and remove barriers to care by facilitating solutions and linkage to needed resources. Case Managers can help clients gain access to the following:
PrEP prescriptions and medication
Confidential Rapid HIV Testing and Counseling
ART Prescription and Adherence
HIV Prevention Education Sessions
Confidential Partner Testing
CCHC’s primary health care
Oral Health Care
Mental Health Care and Psychiatry
Medication Assisted Treatment (MAT) and other Substance Use Disorder (SUD) treatments
Domestic Violence Services and other support groups
External Referrals
External referrals include rental assistance, food, transportation, finance, housing, legal services, and medication assistance, premium assistance, or assistance with any factors creating a barrier to care. Case Managers also work to ensure continuity of care and retention in care through transportation referrals for transit cards and taxi services.
Case Management focuses on medical compliance and medication adherence, risk reduction, reducing viral loads, and any additional services. Case Managers contact clients one month before their recertification expires to alert them to complete recertification.
