CCHC is licensed by the Division of Substance Use Prevention and Recovery (SUPR) and has received HRSA funding to expand our behavioral health services to include a Medication Assisted Treatment (MAT) program for individuals who develop a dependence on opioids or alcohol. This expansion is an effort to address the national Opioid epidemic as well as the substance use disorder treatment needs of the communities we serve.

CCHC’s MAT Care Team consists of a medical provider (prescriber), a Substance Abuse Counselor, and a Community Health Worker who will work together to help patients and clients create and execute a plan to end their use of opioids and alcohol and lead to a healthier life.

How MAT Can Help

• Prescribing and monitoring of medications such as Vivitrol® and Suboxone® that are used to treat symptoms of withdrawal and craving
• Provides Substance Abuse Group and Individual Counseling
• Provides education on health concerns and support in reaching patient treatment goals
• Connections with other needed services both internally and externally
