SPNS HIV Peer Care Engagement Demonstration Project for Minority Men who have Sex with Men is a 2-year demonstration care engagement project for participants who are not virally suppressed or out of care. The purpose of the project is to increase rates of engagement and retention of newly diagnosed HIV-positive African-American men who have sex with men (BMSM). The goal is to stabilize 75-80 hard-to-reach clients from low income, underserved, predominantly African-American communities on the far southside of Chicago and the surrounding south suburbs; who are aware of their HIV status and who have fallen out of care, never entered into care, or are at risk of falling out of care, and retain them in consistent care for improved viral loads and overall positive health outcomes. CCHC is working with an Evaluation and Technical Assistance Provider (ETAP), funded separately, to evaluate programmatic, clinical, and client-level outcomes of the focused case management intervention seeks to engage to increase retention in HIV care for improved HIV health outcomes.
