Founded in 1991, Christian Community Health Center (CCHC) is a Federally Qualified Health Center (FQHC) and 501 (C)3 non-profit organization that has been providing high quality primary care, oral care and behavioral health services for over 27 years. While primary medical care is CCHC’s anchor program, CCHC has become equally renowned for its presence as a multi-pronged social service agency. CCHC strives to meet the holistic needs of the communities they serve by incorporating primary medical treatment and a comprehensive slate of supportive programs. CCHC has Level III National Committee for Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) recognition and are a Division of Substance Use Prevention and Recovery (SUPR) licensed Level I and II provider of outpatient substance use disorder treatment for adolescents and adults.

Christian Community Health Center (CCHC) is a 501(c)3, federally qualified health center established in 1991 to address the health care and social service needs of Chicago.
CCHC expanded its reach to the South Suburbs of Chicago with new clinic locations in Calumet City (2005) and South Holland (2007).
CCHC is a leader in care coordination, with programs that provide services across an integrated spectrum of areas including Health & Prevention; Education & Life Skills; Housing & Shelter, and Intervention & Support. We currently operate seven sites throughout the Chicagoland area, including three community health centers, as well as a mobile health van providing a wide range of primary medical care and social services.
By incorporating primary medical treatment with an array of comprehensive supportive programs, CCHC strives to meet the holistic needs and all-around well-being of the communities we serve.
