Community Advisory Council

We want your involvement at Christian Community Health Center. We welcome all clients who wish to participate!

What We Do

CAC meets to discuss ways to improve programs and services at Christian Community Health Center on a regular basis.

The Prosumer provides a biannual report to the Board of Directors and meets quarterly with our CEO to provide feedback on ways to improve patient satisfaction and quality of services. Through these avenues, the CAC shares suggestions for improved services with CCHC’s leadership.

New members will be selected through an application process. Potential applicants will submit an application to be screened and interviewed by the Officers and designated CCHC staff. The CCHC CAC will aim to maintain a membership that reflects a wide range of health-related services (e.g., acute/chronic, preventative, and supportive).


Our History

In April 2009, Christian Community Health Center established a Community Advisory Council (CAC) with membership of 5 to 13 members from our HIV care programs, chaired by our Prosumer who leads and coordinates CAC meetings. The purpose of the CAC is to provide input and feedback into program planning and represent CCHC at identified community groups. The CAC includes participants from each of CCHC’s programs and unique populations effected by the HIV epidemic. The planning body engages in focus groups, customer satisfaction surveys, and strategic planning, playing an integral role in ensuring that our patients living with HIV are empowered to participate in the design, implementation, and evaluation of CCHC’s Ryan White programs. The CAC assists with community education, outreach efforts, and supports individuals living with HIV/AIDS.

During meetings, CAC members discuss community and individual needs to inform decisions and programming at Christian Community Health Center. Member input is integral to our work in and for our South Side and Suburban service areas.

CCHC receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals. We have achieved NCQA Level 3 Patient Centered Medical Home recognition at all three clinic sites. We are IDHS-SUPR licensed to provide Level 1 and 2 outpatient substance use treatment services.

This Health center is a Health Center Program grantee under 42 U.S.C. 254b, and is a deemed Public Health Service employee under 42 U.S.C. 223(h).
