Christian Community Health Center works with a number of trained professional staff members who are committed to working together to provide you with quality services. In addition to your medical provider, our health and wellness team members include a nurse and medical assistant and may also include: registration staff, behavioral health therapist, counselor, patient health advocate, or case manager—depending upon your needs. You’re at the center of your care. Our team will give you the tools to help you manage your own health.

Victor L. Thomas, MD. Chief Medical Officer - Internal Medicine and Pediatrics

Dr. Thomas received his Medical degree from the University of Illinois at Chicago School of Medicine in 1987. He completed his residency at John H. Stroger, Jr. Hospital of Cook County in 1991. Since that time, he’s offered 23 years of quality health services in Internal medicine and Pediatrics at Mount Sinai Hospital, Advocate Bethany Hospital, Sacred Heart Hospital, and more. 

Craig Irvine, D.D.S. Dental Director

Dr. Craig Irvine pursued his passion for community health dentistry as an undergraduate student at Southern Illinois University - School of Dental Medicine where he earned his degree in 2011. With over 5 years of experience in oral health (including mobile dentistry within the Chicago Public School System), Dr. Irvine brings a modern and innovative approach to Christian Community Health Center’s Dental Clinic. He is a proud member of The Illinois Public Health Care Association and The National Oral Health Association and has continued servicing the community with a high-quality patient-centered standard as a member of CCHC’s medical team.

Philip C. Okwuje, MD, FACOG Obstetrics Gynecology

Dr. Okwuje is a skilled Obstetrician & Gynecologist who has 39 years of practice experience. He is a fellow of the American College of Obstetrics and Gynecologists and is also a proud member of the American Medical Association, Illinois Medical Society, and Chicago Medical Society. Dr. Okwuje is affiliated with several hospitals including: Advocate Trinity Hospital, Provena Mercy Medical Center, Advocate Illinois Masonic Medical Center, and St. James Hospital – Olympia Fields, IL. His dedication to serving his patients well makes him a great addition to the CCHC medical team.

Dannon R. Martin, MMSc-PA-C, MPH, AAHIVM Family Practice Physician Assistant

Physician Assistant (PA) Dannon Martin graduated from Emory University in 2014 with a Master of Medical Science in Physician Assistant Studies degree. She has also earned a Master of Public Health degree from Morehouse School of Medicine (2011) and a Bachelor of Science in Biology from Spelman College (2009). PA Martin, a certified HIV specialist, has accrued meaningful professional experience promoting healthy lifestyles to various special populations in community based healthcare settings. Her educational background coupled with her foundational work in public health provides for a well-rounded modernized approach to her healthcare practices.

Leslie D. Moore, RN, CNM, MSPH Certified Nurse Midwife

Nurse Leslie Moore is a Certified Nurse Midwife who completed the BS-RN program at the University of Illinois at Chicago in May 2010. She gained valuable clinical experience practicing as a Registered Nurse with various populations and went on to earn her Master of Science in Nursing in May 2013. As a Certified Nurse Midwife, Nurse Moore aims to work with women from various backgrounds to optimize their health and wellbeing in a manner that integrates mind, body, and soul across the lifespan in the women’s health specialty which also includes pre-natal and post-partum care.

Angel Leake, MD, MBA Infectious Diseases Physician

Dr. Angel Leake has been practicing as a licensed physician since 2008. After she earned her Doctor of Medicine degree from the University of Illinois College of Medicine in 2002, she completed several years of hands-on residency and fellowship training at Mercy Hospital and Medical Center and McGaw Medical Center of Northwestern University respectively. Dr. Leake specializes in infectious diseases and is a proud member of the Infectious Diseases Society of America, the HIV Medicine Association, the Illinois State Medical Society, and The American College of Physicians.

Dorothy Akaeze, NP Family Nurse Practitioner

Nurse Dorothy Akaeze is a Family Nurse Practitioner who has 9 years of direct patient care experience as a registered nurse. She has held administrative positions such as Nursing Supervisor and Charge Nurse during her RN practice history and exemplifies leadership in nursing practice and a thorough knowledge of providing quality patient-centered care. Nurse Akaeze received a Master of Science in Nursing degree from Governor State University in May 2015 and has been successful in implementing the professional skills acquired as an RN to her career as a Family Nurse Practitioner.

Amy Ramirez, M.D. Pediatrician

Dr. Amy Ramirez received a Bachelor of Science degree from the University of Illinois at Chicago in June 1971, and a medical degree from Northwestern University’s Feinberg School of Medicine in 1976. She brings 40 years of experience to the CCHC medical team and looks to continue providing quality patient care that focuses on the medical and psychosocial aspects that affect pediatric health. Dr. Ramirez is bilingual speaking both English and Spanish and is a proud member of the American Academy of Pediatrics.

Solomon Okai, MD Internal Medicine

Dr. Solomon Okai brings 30 years of valuable experience as a medical provider to CCHC’s patient care team. He earned his Bachelor of Science degree from University of Nevada, and attained his medical degree from University of Southern California specializing in Internal Medicine. Dr. Okai blends compassion with his extensive knowledge of medical practices to provide patients with a quality experience that addresses their needs holistically. He is a wonderful addition to our medical staff and is committed to servicing the community while upholding the patient-centered standard.

Caryn Woods, NP - Family Nurse Practitioner

Nurse Caryn Woods is a Family Nurse Practitioner with a passion for enhancing the health of her patients by providing quality care. She earned a Bachelors of Science in Nursing from the University of North Carolina in 2009 and practiced as a registered nurse for years before pursuing a Masters of Science in Nursing which she earned from Governors State University in 2015. Nurse Woods is a proud member of the American Academy of Nurse Practitioners and is certified by the American Heart Association.

Lilnetra Grady, ANP, MS, Advanced Practice Nurse - Family Practice

Nurse Lilnetra Grady received a Master of Science in Nursing from Olivet Nazarene University (2016) and a Bachelor of Science in Nursing from Rockford College (2001). Nurse Grady worked with CCHC as in intern for 6 months in training prior to joining CCHC as a medical provider. She currently provides Direct Primary Care services to patients living with HIV/AIDS. Nurse Grady has been with CCHC since November 2016.

Edwin Ekong, D.O., MPH, FAAFP - Internal Medicine

Dr. Edwin Ekong has over 36 years of experience as a physician. His extensive family practice background includes healthcare services he's provided at numerous health facilities including Northwestern General Hospital and Wayne State School of Medicine both in Detroit, Michigan. Dr. Ekong has also practiced at Chicago College of Osteopathic Medicine, Mt. Sinai Hospital System and Urban Health Center in Chicago, and West Court Medical Center in Kankakee, IL. Dr. Ekong has a Bachelor of Science in Pharmacy from Wayne State University and a Doctor of Medicine degree from Michigan State University. He also holds several certifications with the National Board of Examiners and the American Family Board of Osteopathic Family Practice. He is a member of the American Academy of Family Practice, the Illinois Academy of Family Practice, and the National Medical Association. Dr. Ekong recently joined CCHC in July 2017.

Diane Washington, MD, - Psychiatrist

Dr. Washington has over 20 years' experience providing psychiatric assessment and treatment for patients with mental health disorders, including management of substance abuse and dual diagnosis. Dr. Washington has experience working with adults, adolescence, and geriatric patients to develop and implement innovative treatment options and alternatives. Dr. Washington, who joined CCHC in July 2017, completed her Internal Medicine internship at Michael Reese Hospital and Medical Center in 1989, her residency in General Psychiatry in 1993 at the University of Chicago Medical Center. She is a member of several associations including the National Medical Association, the Black Psychiatrists of America and the AIDS Legal Council of Chicago.

Loudjine Jean-Michel, DDS - Dentist

Dr. Loudjine Jean-Michel earned her Doctor of Dental Surgery degree from the University of Detroit Mercy (2011). Prior to that she earned a Bachelor of Science degree from Loyola University Chicago. Throughout her over five years of experience she remains dedicated to the prevention and progression of dental disease. Currently, using the latest research and a modern approach, she remains dedicated to sharing her skills with the community of CCHC.

Sandre L. Crain, MSN- Advanced Nurse Practitioner

Nurse Sandre Crain received her Bachelor of Science in Biological Sciences from the University of Illinois at Chicago in May 2001. She worked in medical labs for nine years where she gained valuable experience testing samples and assessing specimens collected from patients. She then earned a second Bachelor of Science degree in Nursing in May 2013 (Purdue University), and went on to achieve her Masters of Science in Nursing in May 2017 from Olivet Nazarene University (Bourbonnais, IL). Nurse Crain brings her advanced nursing skills and expertise to our patient care team with a focus to provide quality compassionate comprehensive care to the CCHC community.

Andre Dejean, MD- Internal Medicine

Dr. Andre Dejean brings over twenty years of medical experience to CCHC’s patient care team. He earned a Bachelor’s Degree (1984) and a Master’s Degree (1989) in Biology, and continued his education to earn a Doctorate of Medicine in 1994 from the University of Illinois at Chicago. He’s provided his medical expertise at the likes of Presence Medical Group, MetroSouth Medical Center, St. Bernard Hospital, and more. Dr. Dejean has shown commitment to excellence by pursuing numerous hours of continuing professional education in the medical field and is a proud member of the American Academy of Family Physicians.
